Southwest Washington Regional Surgery Center in Vancouver suffered a data breach incident in September 2018. The breach exposed some Protected Health Information, including some patients’ names, Social Security numbers, driver’s license numbers, and/or medical information (diagnosis, treatment, surgery, medications, labs and/or health insurance information) of 2,393 patients. SWRSC offered a credit monitoring and identity theft restoration service to protect their information.

The Washington State Office of the Attorney General reported on November 6, 2018, that Southwest Washington Regional Surgery Center, LLC experienced a data breach due to a phishing cyberattack, affecting 1,690 residents. The breach occurred between May 27, 2018, and August 13, 2018, with the compromised employee email account potentially exposing personal information including names, Social Security numbers, and medical information.
